Welcome!  The purpose of this site is to allow you to take the Simmons Personal Survey. 

In order to take the Survey, you have been directed to this page by an authorized company representative, and should have been given an account number and a password by that person.

* If you have an account number and a password, you are ready to take the Survey.  

* If you do not have an account number and a password, contact your company representative.    

* Use the Internet Explorer browser.  Other browsers may not work.

Note: the Account Number is always nine numbers (using  a  0, 1, 2, 3, 4, 5, 6, 7, 8, or 9).  If the number you have is less than 9 digits, key in zero's before your number, to have a total of 9.  For example 000001234.
